Bedsores, also known as pressure ulcers1, are painful wounds that result from prolonged pressure on certain areas of the body. They’re most common in people who have medical conditions that cause them to spend a lot of time reclining or sitting in the same position.
According to our Chief Medical Advisor, Dr. Raj Dasgupta, a mattress must showcase a few key traits to effectively prevent bedsores. Namely, it should distribute the sleeper’s weight evenly and provide enough support.
“The mattress needs to distribute weight evenly to prevent having too much pressure in one area of the mattress,” he said. “It should also be supportive enough to keep your body properly aligned while sleeping, which will also help to reduce the likelihood of pressure points that can contribute to ulcers.”
Many kinds of specialized mattresses have been designed to help prevent bedsores from developing or worsening. For people who are largely bedridden and often in hospital-grade beds, alternating pressure mattresses or mattress toppers are a good option for keeping pressure ulcers at bay1. These electrically powered mattresses steadily inflate and deflate different chambers inside of them to keep changing the distribution of pressure on the user’s body.
However, traditional mattresses can also help with bedsore prevention, and many people find them more comfortable than alternating pressure mattresses. As Dr. Dasgupta concluded, “The best way to choose a mattress specifically for pressure ulcer prevention is to try out different mattresses and see what is most comfortable for that person.” He also advised that anyone at risk for bedsores consult a medical professional as they shop for mattresses, then reevaluate their condition and comfort regularly.

The bed is medium-firm for a middle-of-the-road feel. The top layers cradle the body without much risk of overheating, leading to a drier surface (which should also help to stave off pressure ulcers).
What’s the Helix Midnight Luxe Made Of?
The Helix Midnight Luxe stands 13.5 inches tall and is composed of 6 layers. All of the foams used in the Helix Midnight are CertiPUR-US® certified. You can pick between two covers: a breathable Tencel™ cover or a GlacioTex™ cooling cover. Both are soft and permit cooling airflow.
Underneath, there are three layers of foam. The first is a layer of Helix Responsive Foam, then a layer of copper gel memory foam to balance cooling, and then a final layer of Memory Plus Foam for stability.
The innerspring unit is made up of individually wrapped coils and is zoned with stronger coils in the middle to support the hips and lower back.
The coils around the edges of the bed are also reinforced for a sturdier perimeter. Beneath the springs, you’ll find a base layer of dense foam.

What We Liked
- Supportive but soft – The combination of coils and cushiony top layers make the Helix Midnight Luxe great at providing comfortable lift. The medium-firm feel of the mattress should balance support and softness.
- Versatile – This same balanced feeling mentioned above makes the mattress great for most sleeping positions and body types. It’s also responsive enough that you should be able to switch positions with ease.
Potential Drawbacks
- Not for heavier stomach sleepers – Though this bed should provide support for a number of sleepers, some plus-size stomach sleepers may still find it too soft for their liking. They need a mattress with even more support around the hip region.

Why the DreamCloud Earned Best Hybrid Mattress for Preventing Pressure Ulcers
DreamCloud consistently ranks as one of my top favorites, but I also believe in its capabilities when it comes to bedsore prevention. It has a firm pillow top that should promote even weight distribution, while at the same time balancing support and that all-too-necessary pressure relief.
It also offers stellar motion isolation, especially for a hybrid, thanks to its thick memory foam layers and pocketed coils. That’s an important trait if you share a bed, but also if you’re concerned about bedsores and don’t want too much jostling around key pressure points.
What’s the DreamCloud Made Of?
The DreamCloud is a whopping 14 inches thick. It’s made up of 7 layers including both a top and bottom cover. The top cover is made of a luxurious cashmere blend that is woven to feel plush and last long.
Next comes three layers of CertiPUR-US® Certified foams: a 1.5-inch plush foam for cushioning, a 1-inch gel-infused memory foam for pressure relief, and 2 inches of supportive dynamic foam.
Beneath these is the coil unit of the bed, 8.5 inches of individually wrapped coils. Thicker coils line the perimeter to bolster the edges of the bed.
All of this sits atop a foam base. Finally, the bottom cover is designed to resist shifting and has four handles for ease of movement.
My Take: I like calling the DreamCloud a “jack of all trades” because it packs a lot of features into one mattress and feels good in any sleeping position, all at a reasonable price.
What I Liked
- Affordable luxury – The cashmere blend on top of this bed feels like a dream, and when it’s combined with the many layers and thick profile, it’s difficult to believe that the DreamCloud doesn’t cost as much as its competitors.
- Responsive surface – The DreamCloud’s hybrid construction creates a bounciness that tossers and turners should appreciate. Its coils should aid in bounce-back when moving around.
Potential Drawbacks
- Not the most contouring – If you’re looking for the typical “hugging” memory foam feeling, check out our other picks on this list, particularly the all-foam options.
- Lackluster edges – Despite the DreamCloud having thicker coils around its perimeter, I thought there was still something to be desired from its edge support. Be aware if you’re someone who prefers to sleep or sit at the very edge of your bed.

The DreamCloud Original scores a solid 4.7 out of 5 stars among customers on its site. One potential downside of the DreamCloud Original mattress is its weight, making it challenging to move or rotate, but it offers excellent support and comfort for those who prefer a medium-firm feel.

The Plank Firm is a flippable bed that lives up to its name as one of the firmest mattresses you can find on the online market. When it comes to mitigating pressure ulcers, the mattress definitely has even weight distribution down pat.
We recommend the medium-firm side for a smidge more pressure relief, but both sides should feel firm and supportive. For an added fee, you can also get a cooling cover to regulate temperature and moisture buildup from sweat.
What’s the Plank Firm Made Of?
The Plank Firm packs four distinct layers into 9.5 inches of mattress. It’s flippable and split into two sides: an extra-firm side and a firm side. The center core consists of 6 inches of high-density foam that gives the bed its support.
The extra-firm side only has a quilted top above the foam core, and it’s stitched to provide absolutely no lift for a seriously firm experience. On the other hand, the firm side has a quilted top layer with a small amount of loft that makes it feel more traditionally firm. Underneath is a layer of TitanFlex™ foam for bounce and pressure relief.
My Take: In my opinion, Plank beds are some of the best when it comes to even weight distribution. Folks looking for this quality plus a firm surface should be satisfied with what the Plank Firm has to offer.
What I Liked
- Flippable – This mattress has two sides, a traditionally firm side and a super-firm side. Depending on how much give you want from your mattress—some or none at all—you can easily flip between the two.
- Endorsed by experts – The Plank Firm’s ability to evenly distribute weight while maintaining spinal alignment earned it an endorsement from the American Chiropractic Association.
Potential Drawbacks
- Too firm for some – Because pressure relief is also important for preventing bedsores, I want to be very clear on this point: When I say the Plank Firm is firm, I mean it! Only those who want a truly firm bed should dare to try it.
- Cooling costs extra – The standard Plank Firm comes without any added cooling features. But you can rectify this for an added fee, which nets you top and bottom covers augmented with GlacioTex™ cooling fibers.

Its thick, pillowy upper portion lends itself well to pressure relief and features advanced proprietary foams. Meanwhile, the coils underneath provide support and airflow. What’s the Nolah Evolution Made Of?
The Nolah Evolution has nine layers (including its upper and lower covers) and stands 15 inches high. You can order it in one of three firmnesses: Plush, Luxury Firm, or Firm. We recommend the Luxury Firm, as a medium-firm profile should be most effective for preventing sores.
The cover on the Evolution is made of organic cotton and ArcticTex™ fibers, the latter of which help to absorb and disperse heat. Underneath is a Euro topper quilted with 2 inches of foam. The quilting is labeled HTC for high thermal conductivity, implying that it also assists with redirecting unwanted warmth.
The topper is encased in a gusset with vents along the sides that encourage heat to exit the bed.
Next is a 2-inch layer of AirFoamICE™, Nolah’s proprietary cooling foam. The brand maintains that this material easily outcompetes memory foam in terms of pressure relief, durability, and temperature regulation.
It’s followed by 2 inches of support foam, and another inch of transitional foam that buffers the comfort layers above from the spring unit below.
The coils in the Evolution are split into three zones that give customized amounts of support to different areas of the body, including an especially supportive set of springs for the lumbar region of the back. Along the bed’s edges, the coils are reinforced for extra resistance to dipping and sagging.
Beneath the spring unit is a final and foundational layer of recycled plant fiber.

What We Liked
- Loads of pressure relief – “When I crawled into this bed, I could really feel it gently cradle my body,” said our tester Stuart. Liz echoed this point: “I really love the relief I get from the pressure in my hips.”
- Awesome edge support – “I really love the care Nolah has taken to reinforce the edges,” said Liz. Getting in and out of bed didn’t feel destabilizing, and lying on the edges was likewise worry-free.
Potential Drawbacks
- It’s bulky – “The Nolah [Evolution] is a pretty large mattress, so make sure you ask for assistance when unboxing,” said Stuart. By the same token, if you ever need to move the bed to another room, you’ll likely need another pair of hands.

What’s the Saatva Solaire Made Of?

If you order an Upper-Flex model (available in king or queen sizes), the mattress will have upper portions that can elevate independently of one another with the help of an adjustable base, as well as dual air chambers and two remotes. Potential Drawbacks

What to Look for In A Mattress for Pressure Ulcers and Bedsores
Bedsores can be dangerous if not treated with care and consideration1. There are many features to keep in mind when searching for an accommodating mattress; we dive into the details to help you know what you’re looking for.
Type of Mattress
Multiple mattress models are available for pressure ulcers; typically the best ones use bubble cells or alternating air pressure. You’ll want to carefully research2 how you or your loved one will be using the mattress, what the specific need is — preventative or treatment — as well as your budget.
Dr. Dasgupta recommends that you consult your doctor before you shop, as well. Keep in mind that just because a mattress has a certain label or qualification doesn’t mean it will always be the best bed for your needs.
Alternating Pressure Air Mattress
Air beds that use alternating air work by a connected series of air bubbles that are alternately inflated and deflated3 to prevent any one area from becoming overexposed to pressure. These types are also fantastic for minimizing the amount of moisture buildup that gathers around the body, which could exacerbate sores.
Dr. Dasgupta explained that “Alternating Pressure Mattresses (APMs) offer several advantages, including their effectiveness in preventing pressure ulcers, reducing the risk of friction and shearing forces, and improving circulation, which helps lower the risk of skin breakdown.” Despite all these helpful qualities, they tend to be pricier than traditional foam mattresses and may feel less comfortable to some people.
Low Air Loss Mattress
Another excellent option when choosing a mattress for bedsores is a low air loss4 product. These beds are powered by a blower, which works to circulate air throughout the mattress continuously.
Low air loss products are great for keeping patients dry, and these types are considerably more delicate, making them a great choice for individuals with susceptible conditions. These models can run at a higher cost and are often thought to be more challenging to set up, but they are considered more comfortable and conducive to healing.
High-Specification Foam Mattresses and Advanced Static Mattresses
High-specification foam mattresses and advanced static mattresses are two other types of mattresses often mentioned in bedsore research.
The former term signifies that the mattress has been made with high-quality foam and possesses features designed to help with pressure ulcers. Some of these features may include:
- Thick foam that conforms to the body and distributes pressure evenly.
- A moisture-wicking cover that prevents the skin from getting wet.
- Minimal motion transfer, so sleepers don’t slide or shift around while sleeping.
Similarly, advanced static mattresses provide pressure relief without the use of air cells—usually by implementing high-density foam, multiple layers of foam, or foam in conjunction with other materials. A high-specification foam mattress can also be an advanced static mattress, and vice versa.
Dr. Dasgupta cautioned that “not all mattresses that are labeled as “high-specification” or “advanced” will be effective in preventing pressure ulcers.” As always, it’s important to seek guidance from a medical professional when deciding on a new bed for someone at risk of developing bedsores.
Traditional Mattress
Traditional mattresses can also be used when trying to eliminate or prevent pressure ulcers.
What kind of traditional mattress is best for preventing bedsores, you might ask? The answer is largely personal, but foam beds tend to be better than hybrid ones. Per Dr. Dasgpupta, “In general, all-foam beds are more effective at helping prevent pressure ulcers than hybrid beds due to their pressure redistribution capabilities.
However, hybrid beds can be a good option for those who prefer the feel of springs or need extra support.”
The American College of Physicians5 also recommends traditional mattresses made of foam or gel that cradle you and don’t move much, as this could help reduce friction.
Sleeping Position
Bedsores are common in those who are bedridden, in a wheelchair, or otherwise unable to sense pain6. For those with or at risk for pressure ulcers, it is recommended that they reposition frequently7 to diminish the chances of obtaining bedsores.
The National Pressure Injury Advisory Panel (NPIAP)8 has specific recommendations for how side sleepers can reposition to prevent pressure ulcers, for example.
Weight/Body Type of Sleepers
When loved ones become immobile and are bound to a bed or wheelchair, weight gain is typical due to the lack of exercise and movement. Once you realize they need a particular mattress to prevent or treat ulcers, be sure to check the weight capacity and account for possible weight gain. For example, if they weigh 160 pounds, get one that supports 260 pounds to be safe.

Firmness
When you have pressure sores, the best thing you can do is stick with a mattress firmness level that offers the most pressure relief for your sleep position. However, this does not mean you should run right out and get the softest mattress you can find.
Getting a too-soft mattress for your sleep position can leave you with poor body alignment, which could lead to back pain.
Generally speaking, you should try to stick to the mattress firmness level that most closely matches your sleep style and body type, and perhaps go down one firmness level to account for your increased need for pressure relief.
Cooling
Although cooling mattresses may not directly affect pressure ulcers and bedsores, they can provide overall comfort. Given that many people who develop these types of wounds spend much, if not all, of their time in bed, mattresses with cooling gels and breathable materials like coils, latex, or cotton may help prevent overheating.
Motion Isolation
The American College of Physicians recommends beds made of foam or gel that don’t move much. As such, foam mattresses that prevent motion transfer would likely be a good choice for these people because these mattresses are likely to absorb the kind of motion that could cause friction, which is one of the ways that bed sores occur.
Memory foam is one of the best materials for motion isolation, and if you want a bed with coils, look for ones that are individually wrapped to curb motion transfer.
Pressure Relief
Considering that ulcers and bedsores often develop from a buildup in pressure9, beds that evenly distribute body weight and keep the spine in a supported, neutral position can offer relief to spots where the pressure may typically build up, such as the hips and lower back. However, the amount of pressure relief that is offered will also depend on body weight and sleep position.
For example, lightweight sleepers won’t get as much pressure relief on a firmer mattress as a heavier person would.
It’s important to get a mattress that encourages proper body alignment, too, so there’s less opportunity for pressure points to contribute to the development of ulcers.
Edge Support
If a bed has good edge support, it means that you will feel supported when sitting or sleeping on the edge of the bed. If you feel as if you are falling off when sleeping or sitting on the edge of the bed, then your mattress likely doesn’t have good edge support.
Often, those with bedsores are immobile or incapacitated to some degree. Therefore, good edge support can help provide them with an extra level of security. Furthermore, if they have a nurse or caregiver who manually moves them, good edge support will give these people a larger surface area to work with.
Budget
Some budget mattresses may work just fine considering the specific need or the individual resting on the product; however, keep in mind how it will be utilized. It’s wise to invest in a high-end bed if the damage to the body is already significant. However, if the product is only used preventatively or won’t be used for a long time, the price may not be an essential factor.
How Can A Mattress Help with Bedsores?
A mattress can help prevent bedsores by evenly distributing body weight so that no one spot receives an overly concentrated amount of pressure. This is often challenging with individuals who are bedridden or immobile, so frequent adjustments are critical for prevention.
Various types of mattresses work well for preventing these ulcers.
- Air loss cushions work by simulating a weightless sensation, relieving pressure from the afflicted area.
- Dynamic beds work by inflating bubble cells for support and deflating ones near the infected region for relief4.
- Static mattresses work by using modern foams to distribute weight and reduce the chance of bedsores and prolonged tension5.
As Dr. Dasgupta put it, “Each type of mattress has its own set of advantages and disadvantages, and the choice depends on individual needs and priorities.”
What is the Best Sleeping Position for Bedsores?
Back Sleeping
Back sleepers tend to sleep best on medium-firmness mattresses. These kinds of mattresses typically have good support and some comforting contouring. While many people who experience bedsores are those who are immobile and spend much of their time on their back in bed, it should be noted that many of the places most susceptible to bedsores, such as the tailbone and hips, heels, back of the head, shoulder blades and back of the knees, are areas that will be in direct contact with the mattress if you sleep in this position6.
Side Sleeping
Softer, plusher mattresses tend to work best for side sleepers. This is because these sleepers put a lot of weight and pressure on relatively small areas of the body, specifically the shoulders and hips, and softer mattresses give them much-needed comfort and pressure relief.
This also means that these mattresses could be good at preventing or relieving pressure ulcers.
Combination Sleeping
Frequently repositioning your sleep posture is one of the ways to prevent bed sores. Therefore, combination sleepers, who switch between two or three positions, should be less likely to develop bedsores. These sleepers typically do well on a mattress that falls in the middle of the firmness scale.
Stomach Sleeping
Those who spend a lot of time sleeping on their stomachs will need to be mindful of developing bedsores around their pelvis, knees, and front of the ankles. Additionally, they will need a mattress that is a bit on the firmer side to help keep their hips from sagging too far down as this could lead to lower back pain.
What Causes Bedsores (Pressure Ulcers)?
Bedsores develop from prolonged pressure on a region of the body9. They typically arise in people confined to wheelchairs or beds due to health or mobility reasons. According to Dr. Dasgupta, “Pressure ulcers most often develop on skin that covers bony areas of the body, such as the heels, ankles, hips, and tailbone.”
While these sores are progressive and difficult to treat once developed, the use of a pressure-relieving mattress can assist in preventing them.
Stages of Bedsores
Bedsores get progressively worse but can be identified early if you know how to look for the signs. Below we discuss what to look for6.
- Stage 1
Some individuals begin to complain about an itching or burning sensation. The spot is red and warm to the touch. - Stage 2
The skin around the sore becomes discolored, and the area develops into an open sore or blister. The open wound is often painful. - Stage 3
Due to the damage below the skin, the wound becomes crater-like in appearance, and the pain continues to worsen. - Stage 4
The wounds become considerably large and severely damaged. The muscles, bones, and tendons are now at severe risk, and infection could strike at any moment.

What is the best sleeping position for bedsores?
Resting on your back should help to distribute weight evenly. However, it’s always a good idea to adjust one’s position regularly because several areas of the back such as the shoulder blades, back of the head, buttocks, and others are highly susceptible to bedsores.
There are still ways to mitigate potential sores if you cannot move someone regularly through the night. Another way to prevent bedsores is to use additional pillows or cushions around the shoulders, hips, knees, and ankles to reduce pressure and friction.
Can using a waterbed and an air mattress together relieve pressure ulcers?
While the data on how a waterbed could be used to treat pressure ulcers is limited, there are a few studies12 which have shown correlations in recovery and care. A waterbed’s delicate nature, as well as revolving air pressure in an air mattress, could provide significant relief to individuals suffering with bed sores.

How dangerous can pressure ulcers be?
Bedsores can be extremely dangerous if not detected or left untreated. According to experts, untreated bed ulcers13 could lead to bacteria in the bloodstream, body tissue inflammation, an infection of the bones and joints, pus accumulation, and cancer. Most bedsores do heal when treated, but there’s a possibility that some might never heal completely.
When should a person see a doctor when it comes to bedsores?
According to experts with Mayo Clinic, you should first try to change positions to ease up on pressure around the affected area. If you don’t notice any improvements after 24-48 hours, then you should consult your doctor. Signs of infection should be addressed immediately, though1.
Tips for Better Sleeping with Bedsores
- Maintaining a watchful eye on a loved one’s skin and keeping them comfortable with frequent adjustments is key for helping with sores. If someone needs to rest in bed at all times, shift their position often to reduce the amount of pressure applied to one spot.
- Additionally, make sure they’re kept dry. Moisture can lead to ulcers11, so using a moisture barrier cream to protect against wetness can help keep skin clean and dry. You may need to change their clothes and bedding regularly.
- If your loved one uses a wheelchair predominantly, foams can relieve pressure from sitting in the same position all day.
